Outpatient Guide

Please call 3144989, extension # 2 or 0813 8488 8158 (WhatsApp) or book an appointment here  for appointment before going to the hospital.


For emergency cases, you may go directly to the hospital and proceed to the emergency room. Patient information will be taken by the emergency nurse.


New Patients

  1. Please proceed to Reception Desk
  2. Present government issued identification card (KTP / SIM) and let them know if you would like to register to see a GP or a specialist
  3. Fill out the form with personal information, such as your current address, telephone number, and the next of kin
  4. You will be directed to the waiting area
  5. The nurse will let you know your queuing number and will call you once it is your turn to see the doctor

Old Patient

  1. Please proceed to Reception Desk and tell them your full name and date of birth
  2. Please mention your appointment schedule to the receptionist
  3. You will be directed to the waiting area
  4. The nurse will let you know your queuing number and will call you once it is your turn to see the doctor

General Information

  • Insurance cards and forms must be submitted to the Insurance Assist Desk for processing and confirmation with the insurance company.
  • Previous medical records, test results, referral letters or copies of follow-up instructions regarding medical treatments from other hospitals must be brought when consulting with Abdi Waluyo Hospital’s doctor.
  • Prescription of medicines from the attending physician shall be given to the hospital Pharmacy. The Pharmacist will dispense and explain the dosage and confirm the expiration date of the medicines to the patients
  • All payments of hospital bills including consultation/professional fees of doctors, medicines, as well as radiology and laboratory procedures, must be processed only at the cashier.
